Health Matrix Direct, LLC will retain member and IMR information after the member cancels or discontinues membership with Health Matrix Direct, LLC, or when the IMR is terminated or resigns from Health Matrix Direct, LLC. This information will be protected in the same manner as that of an active member or IMR.
A member or IMR may request in writing at any time, after discontinuing the membership or resignation/termination of IMR, to remove the information from the Health Matrix Direct, LLC database.

Health Matrix Direct, LLC will share member and/or IMR information with its affiliates, related companies and service providers to the extent it is necessary to provide and manage benefits to the member and their family. Unless otherwise permitted by law Health Matrix Direct, LLC will not share with these parties any information collected that is not needed by these parties to provide the benefits to the member. These organizations are not authorized to use member or IMR information for any purposes other than for matter pertaining to Health Matrix Direct, LLC related business. On occasion certain information about you may be shared with carefully selected business partners, including those who may offer services that complement those provided by Health Matrix Direct, LLC or which are believed may be of interest to you.
